How to add an accident form on the Care Diary

Please note that this tutorial is aimed at those settings that make use of the Care Diary feature and would like to learn how to add an accident form from within this feature.

If your setting does not require the entire Care Diary functionality but still wish to add accident forms, you can enable an Accidents only feature as shown in this tutorial

If your setting doesn’t have the Care Diary feature enabled, but would like to start using it, this tutorial explains how you can enable it. 

Accessing the Care Diary

Provided you have the Care Diary feature enabled, you will find it on the main navigation menu on the left.

Adding an accident form

To add an accident form for a child, just click on their Care Diary card and select “Accident”

You’ll then be taken to the accident form where you need to fill in the following details: 

  •  Date and Time (will be defaulted current date and time)
  • A description of how the accident happened. 
  • Body map (optional)
  • First aid that was administered
  • First aider 
  • Additional info (optional)

Adding a body map

You have the option to add a body map to your accident forms which you can do by clicking the “+ Body Map” button. 

This open a separate modal where you can mark on the map where the injury is located. 

Once you’ve indicated on the body map where the injury is, click “Save Body Map” and you’ll be taken back to the main form.

When you have added all the information to your accident form, click “Create”.

Marking an accident form as discussed

When you are ready to discuss this accident with the child’s relative, just head back to the main Care Diary page and you can access the accident forms which are “requiring attention” here.

Select the accident form you are about to discuss with the relative and you can select to either sign a hardcopy or take a digital signature. 

Either option will complete the accident form and the child’s relative(s) will be notified and sent a copy off the form.

Please note, for a child’s relative to receive a copy of an accident form after it’s been “Marked as Discussed” they will need to have “Parental Responsibility” for the child. It’s likely you set this up when you added the relative, but you can also set this up from Configuration > Manage Relatives > Manage Relationships > Edit Relationships.
